Attention families of student-athletes entering grades 6-12! The fall season is quickly approaching, and it's time to begin your FinalForms registration for athletics. Please see some reminders below: - All student-athletes must be registered through FinalForms here: https://mcsdistrict-mi.finalforms.com - Both students and parents are required to electronically sign all athletic forms in FinalForms. - The MHSAA physical form is required and can be scanned into FinalForms. - Physicals must be dated on/after April 15th, 2026. - Both sides of the MHSAA physical form MUST be complete! - Please note: Incomplete forms and missing signatures will be denied. Student-athletes with denied or incomplete forms will not be cleared to participate. - The MHSAA Physical form can be downloaded here: https://millingtonathletics.com/ under the MORE drop-down - All forms must be submitted in full before the first day of practice/tryouts.
